Johnson won his first global championship in 1991 at the global Championships in Tokyo, defeating Frankie Fredericks by an improbable 0.33-second margin in the 200-meter event. In a Spanish restaurant, Johnson and his agent became ill with food illness two weeks prior to the start of the 1992 Summer Olympics.[8] Johnson grew weaker and lost weight. Prior to the Olympics, he was the clear favorite to win the 200 meters, but he was only able to place sixth in his semifinal heat and missed the 200-meter final by 0.16 seconds. Even so, he was allowed to compete in the 4 × 400 m relay, which took first place and broke the previous world mark with a time of 2:55.74.

He took home the 400-meter U.S. championship in 1993 and went on to win the 400-meter and 4 × 400-meter relay world gold.
