Born in Sertãozinho, São Paulo, Militão joined São Paulo FC’s youth academy in 2010. He made his first-team debut during the 2016 Copa Paulista on July 2 in a 2–1 defeat against Ituano. That year, São Paulo competed with an under-20 squad in the tournament. Militão played 11 matches and scored twice, including a goal in a 4–0 win over CA Juventus on September 18, helping secure a spot in the next round.
He made his professional debut in Brazil’s top division on 14 May 2017, playing in a 1–0 away loss to Cruzeiro. Militão featured in 22 league games that season as São Paulo finished 13th. He scored his first top-flight goal on 17 September in a 2–1 win over Vitória. He received a red card in a 1–1 draw against Vasco da Gama on 12 November. Militão played his final game for São Paulo on 5 August 2018, helping the team to a 2–1 victory over Vasco da Gama that propelled them to the top of the league standings.
